fb34e94d21 Support CREATE SCHEMA IF NOT EXISTS. 
Per discussion, schema-element subcommands are not allowed together with
this option, since it's not very obvious what should happen to the element
objects.

e97b8f2da9 Add CREATE TRIGGER, CREATE INDEX, and CREATE SEQUENCE to the list of 
expressions supported by CREATE SCHEMA.

Also added the beginning of some regression tests for CREATE SCHEMA;
plenty more work is needed here.
