0ca3b16973 Add parallelism support for TID Range Scans
In v14, bb437f995 added support for scanning for ranges of TIDs using a
dedicated executor node for the purpose.  Here, we allow these scans to
be parallelized.  The range of blocks to scan is divvied up similarly to
how a Parallel Seq Scans does that, where 'chunks' of blocks are
allocated to each worker and the size of those chunks is slowly reduced
down to 1 block per worker by the time we're nearing the end of the
scan.  Doing that means workers finish at roughly the same time.

Allowing TID Range Scans to be parallelized removes the dilemma from the
planner as to whether a Parallel Seq Scan will cost less than a
non-parallel TID Range Scan due to the CPU concurrency of the Seq Scan
(disk costs are not divided by the number of workers).  It was possible
the planner could choose the Parallel Seq Scan which would result in
reading additional blocks during execution than the TID Scan would have.
Allowing Parallel TID Range Scans removes the trade-off the planner
makes when choosing between reduced CPU costs due to parallelism vs
additional I/O from the Parallel Seq Scan due to it scanning blocks from
outside of the required TID range.  There is also, of course, the
traditional parallelism performance benefits to be gained as well, which
likely doesn't need to be explained here.

7fcb32ad02 Fix incorrect and inconsistent comments in tableam.h and heapam.c.
This commit corrects several issues in function comments:

* The parameter "rel" was incorrectly referred to as "relation" in the comments
   for table_tuple_delete(), table_tuple_update(), and table_tuple_lock().
* In table_tuple_delete(), "changingPart" was listed as an output parameter
   in the comments but is actually input.
* In table_tuple_update(), "slot" was listed as an input parameter
   in the comments but is actually output.
* The comment for "update_indexes" in table_tuple_update() was mis-indented.
* The comments for heap_lock_tuple() incorrectly referenced a non-existent
   "tid" parameter.

6ede13d1b5 Fix concurrent update issue with MERGE.
When executing a MERGE UPDATE action, if there is more than one
concurrent update of the target row, the lock-and-retry code would
sometimes incorrectly identify the latest version of the target tuple,
leading to incorrect results.

This was caused by using the ctid field from the TM_FailureData
returned by table_tuple_lock() in a case where the result was TM_Ok,
which is unsafe because the TM_FailureData struct is not guaranteed to
be fully populated in that case. Instead, it should use the tupleid
passed to (and updated by) table_tuple_lock().

To reduce the chances of similar errors in the future, improve the
commentary for table_tuple_lock() and TM_FailureData to make it
clearer that table_tuple_lock() updates the tid passed to it, and most
fields of TM_FailureData should not be relied on in non-failure cases.
An exception to this is the "traversed" field, which is set in both
success and failure cases.

2252fcd427 Rationalize handling of VacuumParams
This commit refactors the vacuum routines that rely on VacuumParams,
adding const markers where necessary to force a new policy in the code.
This structure should not use a pointer as it may be used across
multiple relations, and its contents should never be updated.
vacuum_rel() stands as an exception as it touches the "index_cleanup"
and "truncate" options.

VacuumParams has been introduced in 0d83138974, and 661643deda has
fixed a bug impacting VACUUM operating on multiple relations.  The
changes done in tableam.h break ABI compatibility, so this commit can
only happen on HEAD.

459e7bf8e2 Remove HeapBitmapScan's skip_fetch optimization
The optimization does not take the removal of TIDs by a concurrent vacuum into
account. The concurrent vacuum can remove dead TIDs and make pages ALL_VISIBLE
while those dead TIDs are referenced in the bitmap. This can lead to a
skip_fetch scan returning too many tuples.

It likely would be possible to implement this optimization safely, but we
don't have the necessary infrastructure in place. Nor is it clear that it's
worth building that infrastructure, given how limited the skip_fetch
optimization is.

In the backbranches we just disable the optimization by always passing
need_tuples=true to table_beginscan_bm(). We can't perform API/ABI changes in
the backbranches and we want to make the change as minimal as possible.

c3953226a0 Remove table AM callback scan_bitmap_next_block
After pushing the bitmap iterator into table-AM specific code (as part
of making bitmap heap scan use the read stream API in 2b73a8cd33),
scan_bitmap_next_block() no longer returns the current block number.
Since scan_bitmap_next_block() isn't returning any relevant information
to bitmap table scan code, it makes more sense to get rid of it.

Now, bitmap table scan code only calls table_scan_bitmap_next_tuple(),
and the heap AM implementation of scan_bitmap_next_block() is a local
helper in heapam_handler.c.

2b73a8cd33 BitmapHeapScan uses the read stream API
Make Bitmap Heap Scan use the read stream API instead of invoking
ReadBuffer() for each block indicated by the bitmap.

The read stream API handles prefetching, so remove all of the explicit
prefetching from bitmap heap scan code.

Now, heap table AM implements a read stream callback which uses the
bitmap iterator to return the next required block to the read stream
code.

Tomas Vondra conducted extensive regression testing of this feature.
Andres Freund, Thomas Munro, and I analyzed regressions and Thomas Munro
patched the read stream API.

d28cd3e7b2 At update of non-LP_NORMAL TID, fail instead of corrupting page header.
The right mix of DDL and VACUUM could corrupt a catalog page header such
that PageIsVerified() durably fails, requiring a restore from backup.
This affects only catalogs that both have a syscache and have DDL code
that uses syscache tuples to construct updates.  One of the test
permutations shows a variant not yet fixed.

This makes !TransactionIdIsValid(TM_FailureData.xmax) possible with
TM_Deleted.  I think core and PGXN are indifferent to that.

Per bug #17821 from Alexander Lakhin.  Back-patch to v13 (all supported
versions).  The test case is v17+, since it uses INJECTION_POINT.

1a0da347a7 Bitmap Table Scans use unified TBMIterator
With the repurposing of TBMIterator as an interface for both parallel
and serial iteration through TIDBitmaps in commit 7f9d4187e7,
bitmap table scans may now use it.

Modify bitmap table scan code to use the TBMIterator. This requires
moving around a bit of code, so a few variables are initialized
elsewhere.

de380a62b5 Make table_scan_bitmap_next_block() async-friendly
Move all responsibility for indicating a block is exhuasted into
table_scan_bitmap_next_tuple() and advance the main iterator in
heap-specific code. This flow control makes more sense and is a step
toward using the read stream API for bitmap heap scans.

Previously, table_scan_bitmap_next_block() returned false to indicate
table_scan_bitmap_next_tuple() should not be called for the tuples on
the page. This happened both when 1) there were no visible tuples on the
page and 2) when the block returned by the iterator was past the end of
the table. BitmapHeapNext() (generic bitmap table scan code) handled the
case when the bitmap was exhausted.

It makes more sense for table_scan_bitmap_next_tuple() to return false
when there are no visible tuples on the page and
table_scan_bitmap_next_block() to return false when the bitmap is
exhausted or there are no more blocks in the table.

As part of this new design, TBMIterateResults are no longer used as a
flow control mechanism in BitmapHeapNext(), so we removed
table_scan_bitmap_next_tuple's TBMIterateResult parameter.

Note that the prefetch iterator is still saved in the
BitmapHeapScanState node and advanced in generic bitmap table scan code.
This is because 1) it was not necessary to change the prefetch iterator
location to change the flow control in BitmapHeapNext() 2) modifying
prefetch iterator management requires several more steps better split
over multiple commits and 3) the prefetch iterator will be removed once
the read stream API is used.

7bd7aa4d30 Move EXPLAIN counter increment to heapam_scan_bitmap_next_block
Increment the lossy and exact page counters for EXPLAIN of bitmap heap
scans in heapam_scan_bitmap_next_block(). Note that other table AMs will
need to do this as well

Pushing the counters into heapam_scan_bitmap_next_block() is required to
be able to use the read stream API for bitmap heap scans. The bitmap
iterator must be advanced from inside the read stream callback, so
TBMIterateResults cannot be used as a flow control mechanism in
BitmapHeapNext().

9bd99f4c26 Custom reloptions for table AM
Let table AM define custom reloptions for its tables. This allows specifying
AM-specific parameters by the WITH clause when creating a table.

The reloptions, which could be used outside of table AM, are now extracted
into the CommonRdOptions data structure.  These options could be by decision
of table AM directly specified by a user or calculated in some way.

The new test module test_tam_options evaluates the ability to set up custom
reloptions and calculate fields of CommonRdOptions on their base.

The code may use some parts from prior work by Hao Wu.

04e72ed617 BitmapHeapScan: Push skip_fetch optimization into table AM
Commit 7c70996ebf introduced an optimization to allow bitmap
scans to operate like index-only scans by not fetching a block from the
heap if none of the underlying data is needed and the block is marked
all visible in the visibility map.

With the introduction of table AMs, a FIXME was added to this code
indicating that the skip_fetch logic should be pushed into the table
AM-specific code, as not all table AMs may use a visibility map in the
same way.

This commit resolves this FIXME for the current block. The layering
violation is still present in BitmapHeapScans's prefetching code, which
uses the visibility map to decide whether or not to prefetch a block.
However, this can be addressed independently.

b1484a3f19 Let table AM insertion methods control index insertion
Previously, the executor did index insert unconditionally after calling
table AM interface methods tuple_insert() and multi_insert().  This commit
introduces the new parameter insert_indexes for these two methods.  Setting
'*insert_indexes' to true saves the current logic.  Setting it to false
indicates that table AM cares about index inserts itself and doesn't want the
caller to do that.

27bc1772fc Generalize relation analyze in table AM interface
Currently, there is just one algorithm for sampling tuples from a table written
in acquire_sample_rows().  Custom table AM can just redefine the way to get the
next block/tuple by implementing scan_analyze_next_block() and
scan_analyze_next_tuple() API functions.

This approach doesn't seem general enough.  For instance, it's unclear how to
sample this way index-organized tables.  This commit allows table AM to
encapsulate the whole sampling algorithm (currently implemented in
acquire_sample_rows()) into the relation_analyze() API function.

87985cc925 Allow locking updated tuples in tuple_update() and tuple_delete()
Currently, in read committed transaction isolation mode (default), we have the
following sequence of actions when tuple_update()/tuple_delete() finds
the tuple updated by the concurrent transaction.

1. Attempt to update/delete tuple with tuple_update()/tuple_delete(), which
   returns TM_Updated.
2. Lock tuple with tuple_lock().
3. Re-evaluate plan qual (recheck if we still need to update/delete and
   calculate the new tuple for update).
4. Second attempt to update/delete tuple with tuple_update()/tuple_delete().
   This attempt should be successful, since the tuple was previously locked.

This commit eliminates step 2 by taking the lock during the first
tuple_update()/tuple_delete() call.  The heap table access method saves some
effort by checking the updated tuple once instead of twice.  Future
undo-based table access methods, which will start from the latest row version,
can immediately place a lock there.

Also, this commit makes tuple_update()/tuple_delete() optionally save the old
tuple into the dedicated slot.  That saves efforts on re-fetching tuples in
certain cases.

The code in nodeModifyTable.c is simplified by removing the nested switch/case.

c35a3fb5e0 Allow table AM tuple_insert() method to return the different slot
This allows table AM to return a native tuple slot even if
VirtualTupleTableSlot is given as an input.  Native tuple slots have knowledge
about system attributes, which could be accessed in the future.
table_multi_insert() method already can modify the input 'slots' array.

The new table AM method free_rd_amcache is responsible for freeing all the
memory related to rd_amcache and setting free_rd_amcache to NULL.  If the new
method is not specified, we still assume rd_amcache to be a single chunk of
memory, which could be just pfree'd.

84c18acaf6 Remove redundant snapshot copying from parallel leader to workers
The parallel query infrastructure copies the leader backend's active
snapshot to the worker processes. But BitmapHeapScan node also had
bespoken code to pass the snapshot from leader to the worker. That was
redundant, so remove it.

The removed code was analogous to the snapshot serialization in
table_parallelscan_initialize(), but that was the wrong role model. A
parallel bitmap heap scan is more like an independent non-parallel
bitmap heap scan in each parallel worker as far as the table AM is
concerned, because the coordination is done in nodeBitmapHeapscan.c,
and the table AM doesn't need to know anything about it.

This relies on the assumption that es_snapshot ==
GetActiveSnapshot(). That's not a new assumption, things would get
weird if you used the QueryDesc's snapshot for visibility checks in
the scans, but the active snapshot for evaluating quals, for
example. This could use some refactoring and cleanup, but for now,
just add some assertions.

60f227316c Fix assertions with RI triggers in heap_update and heap_delete.
If the tuple being updated is not visible to the crosscheck snapshot,
we return TM_Updated but the assertions would not hold in that case.
Move them to before the cross-check.

Fixes bug #17893. Backpatch to all supported versions.

611806cd72 Add trailing commas to enum definitions
Since C99, there can be a trailing comma after the last value in an
enum definition.  A lot of new code has been introducing this style on
the fly.  Some new patches are now taking an inconsistent approach to
this.  Some add the last comma on the fly if they add a new last
value, some are trying to preserve the existing style in each place,
some are even dropping the last comma if there was one.  We could
nudge this all in a consistent direction if we just add the trailing
commas everywhere once.

I omitted a few places where there was a fixed "last" value that will
always stay last.  I also skipped the header files of libpq and ecpg,
in case people want to use those with older compilers.  There were
also a small number of cases where the enum type wasn't used anywhere
(but the enum values were), which ended up confusing pgindent a bit,
so I left those alone.

0245f8db36 Pre-beta mechanical code beautification.
Run pgindent, pgperltidy, and reformat-dat-files.

This set of diffs is a bit larger than typical.  We've updated to
pg_bsd_indent 2.1.2, which properly indents variable declarations that
have multi-line initialization expressions (the continuation lines are
now indented one tab stop).  We've also updated to perltidy version
20230309 and changed some of its settings, which reduces its desire to
add whitespace to lines to make assignments etc. line up.  Going
forward, that should make for fewer random-seeming changes to existing
code.

11470f544e Allow locking updated tuples in tuple_update() and tuple_delete()
Currently, in read committed transaction isolation mode (default), we have the
following sequence of actions when tuple_update()/tuple_delete() finds
the tuple updated by concurrent transaction.

1. Attempt to update/delete tuple with tuple_update()/tuple_delete(), which
   returns TM_Updated.
2. Lock tuple with tuple_lock().
3. Re-evaluate plan qual (recheck if we still need to update/delete and
   calculate the new tuple for update).
4. Second attempt to update/delete tuple with tuple_update()/tuple_delete().
   This attempt should be successful, since the tuple was previously locked.

This patch eliminates step 2 by taking the lock during first
tuple_update()/tuple_delete() call.  Heap table access method saves some
efforts by checking the updated tuple once instead of twice.  Future
undo-based table access methods, which will start from the latest row version,
can immediately place a lock there.

The code in nodeModifyTable.c is simplified by removing the nested switch/case.

19d8e2308b Ignore BRIN indexes when checking for HOT updates
When determining whether an index update may be skipped by using HOT, we
can ignore attributes indexed by block summarizing indexes without
references to individual tuples that need to be cleaned up.

A new type TU_UpdateIndexes provides a signal to the executor to
determine which indexes to update - no indexes, all indexes, or only the
summarizing indexes.

This also removes rd_indexattr list, and replaces it with rd_attrsvalid
flag. The list was not used anywhere, and a simple flag is sufficient.

This was originally committed as 5753d4ee32, but then got reverted by
e3fcca0d0d because of correctness issues.

e9aaf06328 Remove dead NoMovementScanDirection code
Here remove some dead code from heapgettup() and heapgettup_pagemode()
which was trying to support NoMovementScanDirection scans.  This code can
never be reached as standard_ExecutorRun() never calls ExecutePlan with
NoMovementScanDirection.

Additionally, plans which were scanning an unordered index would use
NoMovementScanDirection rather than ForwardScanDirection.  There was no
real need for this, so here we adjust this so we use ForwardScanDirection
for unordered index scans.  A comment in pathnodes.h claimed that
NoMovementScanDirection was used for PathKey reasons, but if that was
true, it no longer is, per code in build_index_paths().

This does change the non-text format of the EXPLAIN output so that
unordered index scans now have a "Forward" scan direction rather than
"NoMovement".  The text format of EXPLAIN has not changed.

4ce3afb82e Refactor how VACUUM passes around its XID cutoffs.
Use a dedicated struct for the XID/MXID cutoffs used by VACUUM, such as
FreezeLimit and OldestXmin.  This state is initialized in vacuum.c, and
then passed around by code from vacuumlazy.c to heapam.c freezing
related routines.  The new convention is that everybody works off of the
same cutoff state, which is passed around via pointers to const.

Also simplify some of the logic for dealing with frozen xmin in
heap_prepare_freeze_tuple: add dedicated "xmin_already_frozen" state to
clearly distinguish xmin XIDs that we're going to freeze from those that
were already frozen from before.  That way the routine's xmin handling
code is symmetrical with the existing xmax handling code.  This is
preparation for an upcoming commit that will add page level freezing.

Also refactor the control flow within FreezeMultiXactId(), while adding
stricter sanity checks.  We now test OldestXmin directly, instead of
using FreezeLimit as an inexact proxy for OldestXmin.  This is further
preparation for the page level freezing work, which will make the
function's caller cede control of page level freezing to the function
where appropriate (where heap_prepare_freeze_tuple sees a tuple that
happens to contain a MultiXactId in its xmax).

1489b1ce72 Standardize rmgrdesc recovery conflict XID output.
Standardize on the name snapshotConflictHorizon for all XID fields from
WAL records that generate recovery conflicts when in hot standby mode.
This supersedes the previous latestRemovedXid naming convention.

The new naming convention places emphasis on how the values are actually
used by REDO routines.  How the values are generated during original
execution (details of which vary by record type) is deemphasized.  Users
of tools like pg_waldump can now grep for snapshotConflictHorizon to see
all potential sources of recovery conflicts in a standardized way,
without necessarily having to consider which specific record types might
be involved.

Also bring a couple of WAL record types that didn't follow any kind of
naming convention into line.  These are heapam's VISIBLE record type and
SP-GiST's VACUUM_REDIRECT record type.  Now every WAL record whose REDO
routine calls ResolveRecoveryConflictWithSnapshot() passes through the
snapshotConflictHorizon field from its WAL record.  This is follow-up
work to the refactoring from commit 9e540599 that made FREEZE_PAGE WAL
records use a standard snapshotConflictHorizon style XID cutoff.

No bump in XLOG_PAGE_MAGIC, since the underlying format of affected WAL
records doesn't change.

4bac9600f0 Harmonize heapam and tableam parameter names.
Make sure that function declarations use names that exactly match the
corresponding names from function definitions.  Having parameter names
that are reliably consistent in this way will make it easier to reason
about groups of related C functions from the same translation unit as a
module.  It will also make certain refactoring tasks easier.

Like other recent commits that cleaned up function parameter names, this
commit was written with help from clang-tidy.  Later commits will do the
same for other parts of the codebase.

0a20ff54f5 Split up guc.c for better build speed and ease of maintenance.
guc.c has grown to be one of our largest .c files, making it
a bottleneck for compilation.  It's also acquired a bunch of
knowledge that'd be better kept elsewhere, because of our not
very good habit of putting variable-specific check hooks here.
Hence, split it up along these lines:

* guc.c itself retains just the core GUC housekeeping mechanisms.
* New file guc_funcs.c contains the SET/SHOW interfaces and some
  SQL-accessible functions for GUC manipulation.
* New file guc_tables.c contains the data arrays that define the
  built-in GUC variables, along with some already-exported constant
  tables.
* GUC check/assign/show hook functions are moved to the variable's
  home module, whenever that's clearly identifiable.  A few hard-
  to-classify hooks ended up in commands/variable.c, which was
  already a home for miscellaneous GUC hook functions.

To avoid cluttering a lot more header files with #include "guc.h",
I also invented a new header file utils/guc_hooks.h and put all
the GUC hook functions' declarations there, regardless of their
originating module.  That allowed removal of #include "guc.h"
from some existing headers.  The fallout from that (hopefully
all caught here) demonstrates clearly why such inclusions are
best minimized: there are a lot of files that, for example,
were getting array.h at two or more levels of remove, despite
not having any connection at all to GUCs in themselves.

There is some very minor code beautification here, such as
renaming a couple of inconsistently-named hook functions
and improving some comments.  But mostly this just moves
code from point A to point B and deals with the ensuing
needs for #include adjustments and exporting a few functions
that previously weren't exported.

Patch by me, per a suggestion from Andres Freund; thanks also
to Michael Paquier for the idea to invent guc_funcs.c.

b0a55e4329 Change internal RelFileNode references to RelFileNumber or RelFileLocator.
We have been using the term RelFileNode to refer to either (1) the
integer that is used to name the sequence of files for a certain relation
within the directory set aside for that tablespace/database combination;
or (2) that value plus the OIDs of the tablespace and database; or
occasionally (3) the whole series of files created for a relation
based on those values. Using the same name for more than one thing is
confusing.

Replace RelFileNode with RelFileNumber when we're talking about just the
single number, i.e. (1) from above, and with RelFileLocator when we're
talking about all the things that are needed to locate a relation's files
on disk, i.e. (2) from above. In the places where we refer to (3) as
a relfilenode, instead refer to "relation storage".

Since there is a ton of SQL code in the world that knows about
pg_class.relfilenode, don't change the name of that column, or of other
SQL-facing things that derive their name from it.

On the other hand, do adjust closely-related internal terminology. For
example, the structure member names dbNode and spcNode appear to be
derived from the fact that the structure itself was called RelFileNode,
so change those to dbOid and spcOid. Likewise, various variables with
names like rnode and relnode get renamed appropriately, according to
how they're being used in context.

Hopefully, this is clearer than before. It is also preparation for
future patches that intend to widen the relfilenumber fields from its
current width of 32 bits. Variables that store a relfilenumber are now
declared as type RelFileNumber rather than type Oid; right now, these
are the same, but that can now more easily be changed.
