aa3c9bad3e First cut to truly async connect: 
- after connect(2), if async the connection is in "connecting" state
	- the first time a request is sent, "connecting" conns are polled
		- in case of success, the request is sent
		- in case still connecting, LDAP_X_CONNECTING is returned;
		  clients are expected to retry later
	- the "async" behavior must be explicitly enabled by setting
	  the LDAP_OPT_CONNECT_ASYNC option

"local" connections need work

ca4a4456e8 LDAP_THREAD_DEBUG revamping, cleanup, bug fixes. 
Expand usage struct with magic numbers and state info.
Add options "noreinit", "nosync", "nomem", "threadID".  Remove "dupinit".
Detect address changes.  Turn off thread ID tracking by default.

d67a2f2044 Move lconn_tls_ctx to ldo_tls_ctx. Otherwise clients cannot set it after 
ldap_initializ'ing an LD and before connecting on it. Really all of the
global TLS options belong in the ldapoptions struct, instead of static vars.

0ea43c9d7d Assume TLS is properly configured if any one of 
keyfile, certfile, cacertfile, or cacertdir is
provided.  Note that TLS can be properly configured

123948bd5e Add thread debugging wrapper thr_debug.c and ldap_thr_debug.h in libldap_r/, 
enabled with LDAP_THREAD_DEBUG (cpp macro and environment variable):

Move any ldap_pvt_* definitions from ldap_int_thread.h to ldap_pvt_thread.h.
#define ldap_int_thread_equal/ldap_pvt_thread_equal instead of tpool.c:TID_EQ.
Define some ldap_debug_*_t types, and LDAP_UINTPTR_T, in ldap_int_thread.h.
ldap_int_thread.h/ldap_pvt_thread.h can now be included multiple times, giving
different results depending on whether libldap_r/ldap_thr_debug.h was included.
Add some cleanup and some preprocessor hacks.
#define LDAP_THREAD*_IMPLEMENTATION in libldap_r/*.c, used by ldap_thr_debug.h.
Add PTHREAD_MUTEX_ERRORCHECK/PTHREAD_MUTEX_ERRORCHECK_NP in thr_posix.c.

bdbda6a231 ber_get_enum() needs int*, not enum*. An enum can have wrong size. 
(Enum _members_ have type int, but the enum's _type_ can be compatible

38a406c8ab ITS#2717, use default SASL_MECH if none was provided. 
Note that the mech stored in the void *defaults structure is useless
because we can't look inside the structure, and the mech list is needed
as input to the SASL library, before any callbacks are called. It seems
the mech doesn't belong in the *defaults struct, and/or it should be
visible and not opaque.

65bfb44e8e Error when user tries to mix command line and default URI components. 
Remove TLS hard as an ldap.conf option as being not a over-writable
default.  The appropriate way to request ldaps:// is use an ldaps://
URI.  A rewrite to only sent hard for the default URIs but ignored
if the application provides it own URI would be acceptable.

bcf7ab26e4 ITS#2465 fix? ber_get_next must read at least sizeof(tag)+sizeof(len) 
which should be at most 8 bytes. However if we read more than the minimum
message length, we have a problem because we steal bytes from any following
message, and there is no buffer mechanism to push back excess data.
The shortest legitimate message is Unbind at 7 bytes, but there shouldn't
be anything following it. Abandon at 8 bytes is next, so always requesting
at least 8 bytes should be safe. Always requesting 9 was a problem.

Please double-check these assumptions...

fabbbafde9 Don't spawn threads based on ltp_pending_count, it doesn't get decremented 
right away due to ltp_mutex still being held. Just see if all open threads
are active... Use cond_broadcast to signal all waiting threads. Make sure
to remove correct ctx from active list after the thread returns.

Patch to allow referrals to be read on synchronous non-search operations.
Treat referrals the same way as MatchDN or ErrorString values.
Store them in the ld structure and provide an option for ldap_get_option
to retrieve them

9a8eadb9de Fix return values, set stack size. (Default is only 64K, far too small.)

c9b7fc6407 New POSIX threads version support. Detects Draft 4,5,6,7,10 (final). 
Should eliminate individual checks for pthread_yield, sched_yield,
and pthread_detach, but they're left in for now as a redundant check.

c2efb8788b when shutting down a server with open connections (back-{ldap,meta}), 
the backend attempts a ldap_unbind() that results in calling
ldap_pvt_sasl_mutex_lock() with a NULL argument, causing a SIGSEGV.
I added a few assert() to catch this (I wonder if this might
be related to ITS#1982 "kill -INT corrupts database").

If I run ldapmodify & co from a script, I don't want to use '-W password'
because the password shows up in the output of 'ps' for everyone,
and I can't pipe the password to 'ldapmodify -w' because -w uses
getpassphrase() which reads from the tty instead of stdin.
So I added '-y file' which reads the password from file.  The programs
exit if the file cannot be read.

[Complete contents of file is used as password.  Use:
	echo -n "secret" > password
to create a file with "secret" as the password.  The -n avoids
adding a newline (which would invalidate the password).  Note
that echo is a builtin and hence its arguments are not visible
to 'ps'.]

I changed ldapmodify, ldapmodrdn, ldapdelete, ldapsearch, ldapcompare.
I did not bother to change ldappasswd and ldapwhoami, because they
prompt for many passwords.  [I fixed up ldapwhoami.]

Rerun autoconf after applying this patch. [Done.]

Note:  I do not know if Windows NT has fstat(), so I set HAVE_FSTAT to
undef in portable.nt.  (fstat() is used to warn if the file is publicly
readable or writeable.)  [I used fstat() to set the buffer size to
read.]

[Note: using the contents of a file extends the tools to support
passwords which could not normally be provided using getpassphrase()
or via the command line.]

b81122d441 Slapadd is changed to include Operational Attributes (entryUUID, entryCSN,
