c8bffb5584 Fix bdb_tool_dn2id_get() - nobody should be calling bdb_dn2id() directly, 
always call through the cache instead - bdb_cache_find_ndn().

e8c58b4e7f Major API change - (SLAP_OP_BLOCKS) All request parameters are 
consolidated into the Operation structure. All reply parameters
are consolidated into the new SlapReply structure. Most operations
now have identical call signatures... Changes are not #ifdef'd,
revert to -r NO_SLAP_OP_BLOCKS if necessary to back out.

c756bf0590 Change - bdb_tool_entry_put always "sorts" entries: 
instead of just calling bdb_next_id, it calls bdb_dn2id to see if the
    current entry already has an ID. If so, it uses it.
  if not, it recurses to see if its parent exists, and does dn2id_add from
    the database suffix back to the current entry.

Implement back-null (/dev/null style backend)
Cleanup some misc warnings ("%lu" format, unused/uninitialized vars,
        ambiguous operator precedence)

323689da57 Added "dbnotxn" config keyword. If present, back-bdb uses DB_INIT_CDB 
(Concurrent Data Store mode) instead of DB_INIT_TXN. Faster, but tends
to impede writers.
