Frank Lichtenheld 70ab9347f8 Remove NTLM support 
Since Microsoft has abandonded this I think it is time
for us to do the same for OpenVPN 2.8.

Leaves a stub ntlm_support in to make cross-branch
t_client.rc easier to maintain.
