Hallvard Furuseth 9574b65648 Fix mdb_mutex_t, add mdb_mutexref_t, drop MDB_MUTEX(). 
Fixes the types of Windows HANDLEs, they became HANDLE*.
MDB_MUTEX() took a magic r/w arg which it's nice to get rid of.
