Kurt Zeilenga 806557a5ad Update notices to use "OpenLDAP Foundation" instead of 
"Net Boolean Incorporated" and drop "Net Boolean" private
label.  This was done per agreement between the Foundation
and Net Boolean.   Long Live OpenLDAP!
