- fix various connection creation/setup concurrency issues
- use shared connection when always idasserting (similar to ITS#4781)

Briefly compare back-bdb and back-ldbm.

Fix spacing typos.  Prefix an octal file mode with 0.
Mention "notags" (new name for "nolang" from the attribute options patch).
