96d3b29968 Skip detach when delv's client never created
run_resolve allocates dns_client_t late, but the cleanup epilogue
called dns_client_detach() unconditionally. When convert_name() or
dns_client_create() failed first, the detach hit a NULL client and
the REQUIRE(DNS_CLIENT_VALID) inside it aborted the process with
SIGABRT instead of a clean error exit.

Guard the detach with a NULL check. Add a digdelv test that runs
delv on a query name whose first label exceeds 63 octets and
asserts the process does not exit 134.

25c9fb54da standardize CHECK and RETERR macros
previously, there were over 40 separate definitions of CHECK macros, of
which most used "goto cleanup", and the rest "goto failure" or "goto
out". there were another 10 definitions of RETERR, of which most were
identical to CHECK, but some simply returned a result code instead of
jumping to a cleanup label.

this has now been standardized throughout the code base: RETERR is for
returning an error code in the case of an error, and CHECK is for jumping
to a cleanup tag, which is now always called "cleanup". both macros are
defined in isc/util.h.

ace3b4eec8 Use ipv6 queries in delv +ns
`delv +ns` invokes the same code to perform name resolution as `named`,
but it neglected to set up an IPv6 dispatch object first. Consequently,
it was behaving more like `named -4`.

It now sets up dispatch objects for both address families, and performs
resolver queries to both v4 and v6 addresses, except when one of the
address families has been suppressed by using `delv -4` or `delv -6`.

dd098a4b3b Change default max-recursion-queries to 50
Changing the default for max-recursion-queries from 100 to 32 was too
strict in some cases, especially lookups in reverse IPv6 trees started
to fail more frequently. From issue #4921 it looks like 50 is a better
default.

Now that we have 'max-query-count' as a global limit of outgoing queries
per client request, we can increase the default for
'max-recursion-queries' again, as the number of recursive queries is
no longer bound by the multiple of 'max-recursion-queries' and
'max-query-restarts'.

6c3f1f09b2 Improve the badcache cleaning by adding LRU and using RCU
Instead of cleaning the dns_badcache opportunistically, add per-loop
LRU, so each thread-loop can clean the expired entries. This also
allows removal of the atomic operations as the badcache entries are now
immutable, instead of updating the badcache entry in place, the old
entry is now deleted from the hashtable and the LRU list, and the new
entry is inserted in the LRU.

Add a quota for SIG(0) signature checks
In order to protect from a malicious DNS client that sends many
queries with a SIG(0)-signed message, add a quota of simultaneously
running SIG(0) checks.

This protection can only help when named is using more than one worker
threads. For example, if named is running with the '-n 4' option, and
'sig0checks-quota 2;' is used, then named will make sure to not use
more than 2 workers for the SIG(0) signature checks in parallel, thus
leaving the other workers to serve the remaining clients which do not
use SIG(0)-signed messages.

That limitation is going to change when SIG(0) signature checks are
offloaded to "slow" threads in a future commit.

The 'sig0checks-quota-exempt' ACL option can be used to exempt certain
clients from the quota requirements using their IP or network addresses.

The 'sig0checks-quota-maxwait-ms' option is used to define a maximum
amount of time for named to wait for a quota to appear. If during that
time no new quota becomes available, named will answer to the client
with DNS_R_REFUSED.

complete removal of isc_loop_current()
isc_loop() can now take its place.

This also requires changes to the test harness - instead of running the
setup and teardown outside of th main loop, we now schedule the setup
and teardown to run on the loop (via isc_loop_setup() and
isc_loop_teardown()) - this is needed because the new the isc_loop()
call has to be run on the active event loop, but previously the
isc_loop_current() (and the variants like isc_loop_main()) would work
even outside of the loop because it needed just isc_tid() to work, but
not the full loop (which was mainly true for the main thread).

Make dns_dispatch bound to threads
Instead of high number of dispatches (4 * named_g_udpdisp)[1], make the
dispatches bound to threads and make dns_dispatchset_t create a dispatch
for each thread (event loop).

This required couple of other changes:

1. The dns_dispatch_createudp() must be called on loop, so the isc_tid()
is already initialized - changes to nsupdate and mdig were required.

2. The dns_requestmgr had only a single dispatch per v4 and v6. Instead
of using single dispatch, use dns_dispatchset_t for each protocol -
   this is same as dns_resolver.

d76ab69772 Attach to the dns_dispatchmgr in the dns_view object
The dns_dispatchmgr object was only set in the dns_view object making it
prone to use-after-free in the dns_xfrin unit when shutting down named.

Remove dns_view_setdispatchmgr() and optionally pass the dispatchmgr
directly to dns_view_create() when it is attached and not just assigned,
so the dns_dispatchmgr doesn't cease to exist too early.

The dns_view_getdnsdispatchmgr() is now protected by the RCU lock, the
dispatchmgr reference is incremented, so the caller needs to detach from
it, and the function can return NULL in case the dns_view has been
already shut down.

Pin dns_request to the associated loop
When dns_request was canceled via dns_requestmgr_shutdown() the cancel
event would be propagated on different loop (loop 0) than the loop where
request was created on. In turn this would propagate down to isc_netmgr
where we require all the events to be called from the matching isc_loop.

Pin the dns_requests to the loops and ensure that all the events are
called on the associated loop. This in turn allows us to remove the
hashed locks on the requests and change the single .requests list to be
a per-loop list for the request accounting.

Additionally, do some extra cleanup because some race condititions are
now not possible as all events on the dns_request are serialized.

b171cacf4f Use a qp-trie for the zone table
This change makes the zone table lock-free for reads. Previously, the
zone table used a red-black tree, which is not thread safe, so the hot
read path acquired both the per-view mutex and the per-zonetable
rwlock. (The double locking was to fix to cleanup races on shutdown.)

One visible difference is that zones are not necessarily shut down
promptly: it depends on when the qp-trie garbage collector cleans up
the zone table. The `catz` system test checks several times that zones
have been deleted; the test now checks for zones to be removed from
the server configuration, instead of being fully shut down. The catz
test does not churn through enough zones to trigger a gc, so the zones
are not fully detached until the server exits.

After this change, it is still possible to improve the way we handle
changes to the zone table, for instance, batching changes, or better
compaction heuristics.
