Clean up OpenSSL/BoringSSL/LibreSSL reference URLs in changelog

Browse source 
Drop the #Lxxx-Lyyy fragments (replaced with prose line numbers) and
unwrap the line-broken URLs so Sphinx linkcheck can validate them.
